Please note that the life insurance values sought on page 8 are face values, or death benefits, rather than cash values. Cash values can be reported on Schedule E where requested. These values should be reported as assets of the person owning the policy, not the beneficiary.

Likewise with retirement assets; retirement assets are to be reported in the column of the owner or participant of the plan, not in that of the beneficiary.
